Upon completion of the application and according to its instructions, the application should be returned either by fax at 401-462-6958, by mail or in person to: Admissions Office, Regan Building, Eleanor Slater Hospital, P.O. Box 8269, Cranston, RI, 02920, with additional documentation enclosed as requested on the application form.

Upon receipt of the application by the Admissions Office, the application will be reviewed by the Admission Assessment Team comprised of a physician, nurse, social worker, physical therapist, and psychologist.

If it is determined by the Admission Assessment Team that the patient is an appropriate candidate for the Eleanor Slater Hospital, the Admission Team will schedule an on-site visit, if necessary, to assess the potential candidate.

Once a candidate meets the criteria for admission to Eleanor Slater Hospital, candidates are placed on a waiting list associated with the specific type of medical services required: Acute, Long-Term, or Adult Psychiatric.
